Supply Reimbursement Form
ONE TIME benefit for members who purchase most of the supplies on a class supply list. You can be reimbursed 50% of the cost of your supplies, up to $50, IF you are new to the media and enroll in a class requiring you to purchase new supplies. Teacher must approve the form, and you must attach receipts.
Membership: Liability Waiver / Welcome Letter / Needs-to-Know
(MEMBERSHIP FORM UPDATED FOR 2026) Required by Robson and by the Club to become a member, register for classes, and use the studio.
You may access and print the Membership Lability Waiver here. Page 1 is the membership form; pages 2 & 3 are a Welcome Letter from the Club President Ginny Hutcheson, and an important Member Needs-to-Know document for safety rules.
INSTRUCTIONS: Fill out and return the liability waiver during QCFAPC studio hours (see Studio Hours on Welcome page) with the appropriate fee (check payable to QCFAPC or EXACT CHANGE only). Print and keep the Welcome Letter and Member-Needs-to-Know for yourself for reference.
