Supply Reimbursement Form
ONE TIME benefit for members who purchase most of the supplies on a class supply list. You can be reimbursed 50% of the cost of your supplies, up to $50, IF you are new to the media and enroll in a class requiring you to purchase new supplies. Teacher must approve the form, and you must attach receipts.
